I've had this happen for three or four games so far. Here's a pyramid to help describe what's happening:

Someone will make slot 1 collapse using tile A. Tile A ends up settling in slot 5. However, slot 1 still shows the graphic for tile A and slot 5 shows no graphic. The next player gets to choose between slot 2 and 6 as A is in slot 5 (even though I can't see a graphic there) so 2 is a valid tile placement. Eventually someone will place a tile B in slot 1. Tile B is then hidden behind the graphic of slot A.
